Job Type: Full-Time (On-site, Homecare & Facility Visits)

Locations: Bucks county & Montgomery county, PA

Schedule: Monday-Friday, 8:00 AM-5:00 PM, with occasional night/weekend on-call needs (schedule flexibility required).

Relocation: Not available. Sign-on bonus offered.

About The Role

We are seeking experienced and compassionate Registered Nurse Case Managers (RNCM) to join a clinician-owned and operated hospice team dedicated to improving quality of life for patients and caregivers. This role is ideal for professionals passionate about providing care to individuals on their end-of-life journey, while working in a collaborative, supportive, and upbeat environment.

Key Responsibilities

  • Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing patient evaluations.
  • Initiate, develop, and update individualized plans of care according to patient needs.
  • Collaborate with hospice physicians and the Interdisciplinary Team to carry out appropriate interventions.
  • Supervise and coordinate the work of home health aides, social workers, and chaplains.
  • Educate patients and caregivers regarding care plans, symptom management, and expectations.
  • Maintain high standards of compliance in documentation and patient care.
  • Support ongoing quality improvement efforts within the agency.

Qualifications

  • Active RN license in Pennsylvania.
  • Minimum of 1 year RN experience (hospice experience preferred, but new graduates considered).
  • Willingness to travel within assigned counties.
  • Valid driver's license, reliable transportation, and liability insurance.
